General LT problem / no problems survey?

Reading the thread by "bockelmann" about "How long they last" brought me the idea of a "general LT survey" and I did not want to hijack his thread.

In case there is no such I propose that we should create one together. It could be a bit similar to the FD poll but with a wider scope. And a little bit more info than in "LT Trivia" poll.

In this survey we could look for for instance: (please feel free to add / correct)

- the registration year of the bike (or maybe model year, but that seems to be a constant source of confusion among American / overseas participants...)
- miles / km
- first / second / third etc owner
- bike ridden a) only summer (max 4 months) b) up to 6 months c) up to 8 months d) year round

- the most common problems listed on each line like:
-- first final drive at ___miles, (do we need to go more details like a) crown bearing b) tapered bearing c) other) second at ___ miles etc.
-- clutch at __ miles, which was caused by a) slave cyl leak b) engine seal leak c) any other common reasons d) don't know
-- FD pivot bearing changed at __ miles
-- reverser problems at ___ miles caused by a) speed sensor b) control unit fault c) reverser relay d) what else e) other / don't know
-- brake problems at ___ miles caused by a) faulty ABS sensor b) low fluid level in reservoirs c) ABS control unit going South d) other
-- brake rotor problems at ___ miles cured by a) new rotor b) new pads c) other
-- valves needed adjustment first time at ___ miles
-- camshaft chain tensioner changed at ___ miles
---
--- etc.

More general info like:
- dropped the bike a) never b) once c) 2- 5 times d) 5-10 times e) constantly
- servicing the bike a) all by the dealer b) oil change and small maint DIY c) previous + engine check + brake pads DIY d) everything DIY
- bike ridden two up a) 0 - 10% b) 10 - 30% c) 30 - 60% d) over 60%
- pulling a trailer a) no b) 0 - 30% c) 30 - 60% d) over 60%

Rider info like year of the birth, location etc.

Now that the current platform LT is turning to a vintage bike, this kind of survey could be nice and useful to create. People with more scientific / statistics background could comment and improve the idea. Perhaps the input of the survey could exported to Excel or something for a deeper approach, I don't know...
Also riders with no issues could answer as usually we only hear about the problem cases and nobody tells that "I've done 60.000 miles without problems".

Please comment or advise if this kind of survey already exists somewhere and who could possibly be the IT-guru to create it on this site.
