4th gear issue

My 99 LTC with 49k will occasionally cut out while in 4th at a steady rpm of around 4500. As soon as I goose it it clears. Only does it in 4th at a steady rpm. Any ideas?

Yes. 200 miles ago with a BMW filter.

Maybe a vent line got pinched during reassembly.

Is the original charcoal canister still installed? Being a 99 model a canisterectomy might be in order.
